This is the place to complete all the forms necessary to compete in Schwan's USA CUP. Please observe the following deadlines:

June 10, 2011

  • »  Permission to Travel, non-Minnesota teams only
  • »  State association, provincial/club or US Club Soccer roster
  • »  Hotel name, location, phone number
  • »  Preliminary dorm info: Number of players/gender, chaperones/gender
  • »  Transportation request
  • »  Player fees

June 15, 2011

  • »  Preliminary roster due

July 14, 2011

  • »  USA CUP Weekend final roster, waivers and late-add player fees

July 17, 2011

  • »  USA CUP Final roster, waivers and late-add player fees

July 1

  • »  Last day for dorm residents to drop from the dorm housing list and still receive a refund
