Medical Facility – NSC Campus
- Steven R. Elias MD, Medical Director Elias010@umn.tc.edu or 651.481.0195
- Location: Schwan Center
- On-site Medical Care. The on-site medical facility at Schwan’s USA CUP is the best that you will find at any soccer tournament in the world. The attention a player receives on site is comparable to that of a local hospital emergency room. On each field cluster there are first aid personnel ready in case of an injury. They evaluate the situation and, if necessary, transport injured players to an on-site medical facility where a staff of volunteer doctors and nurses are available to evaluate and treat injuries. More than 100 of the area’s sports medicine professionals and residents volunteer in the tournament medical facility.
- Transportation. Injured players are transported to the medical facility on golf carts for medical evaluation. Players must be accompanied by a parent or an adult functioning as a legal guardian. Players requesting medical attention without guardianship are treated at the discretion of medical personnel. Return transportation to fields may be requested at Tournament Headquarters. Transportation to a local hospital emergency room, if necessary, will only be provided by ambulance or parents or an adult functioning as a legal guardian. Return transportation is the responsibility of the team.
- ER Evaluation. Players transported to a local ER via ambulance will incur charges for transportation plus hospital charges for evaluation and treatment. The legal guardian accompanying the player to the hospital will need to produce documentation of the player’s insurance carrier, address, policy number, and parent(s) name and address.
- Taping. Taping services are not provided.
- Return to Play. After evaluation, the Schwan’s USA CUP medical staff will recommend whether or not the player should or should not return to play. If they recommend that a player not return to play, medical staff will issue a red medical wristband and request written acknowledgement by the accompanying parent/adult that they understand that Schwan’s USA CUP medical staff has recommended this player not to play.
- Medical Facility Charges. Players evaluated at the tournament’s medical facility are not charged for evaluation and treatment except:
- X-ray studies may be available at on-site medical facility. A flat fee will be charged for on-site x-ray payable with Visa or MasterCard or cash only. A receipt for charges will be given. The individual player/parent is responsible for submitting the charges to their insurance company for reimbursement. This expense may/may not be reimbursed depending on the policy of your insurance company because it is not a registered medical facility.
- Orthopedic supplies, crutches
- Referral to outside consultants
- Pre-authorization. The medical staff will not contact insurance carriers for pre-authorization of charges to be submitted for x-ray supplies. It is the responsibility of the parents to submit these charges for reimbursement. Your insurance may or may not cover charges from the medical facility, since it is not a registered medical facility. Original x-rays obtained on site are not released; these records must be retained by Schwan’s USA CUP medical personnel as part of the medical record. Copies of x-ray studies are available with an appropriate charge.
Roster Guidelines
- Teams may over roster, not exceeding the 6-guest-player-limit. If your roster exceeds the 6-guest-player limit, contact your state association for information regarding reclassification as a tournament team. The 6-guest-player-limit is not negotiable; it is part of the tournament sanctioning. If your team is reclassified as a tournament team, forward the documentation to the tournament.
- Roster sizes:
- U9-U10 teams playing 6v6 may roster 22 and dress 12 players for games.
- U11-U12 playing 8v8 may roster 22 and dress 14 players for games.
- U13-U19 and Adult playing 11v11 may roster 22 and dress 18 players for games.
- If you do not have all the player passes for Final Team Check-in, arrange for your players/guest players to either check in as individuals at Final Team Check-in or report to Tournament Headquarters at least two hours before the team’s first game.
- Players on the roster at Final Team Check-in, but who arrive late, are legitimate players. These players must report to Tournament Headquarters with their player pass at least 2 hours prior to their first game time for approval to play. Remember: Players may not be added to the roster once the roster has been frozen and the game has started.
- Once the first game has started, no players may be added to the tournament roster.
- Players may play up or down, if age eligible.
- Players that are age eligible but registered with an adult league with an adult player pass are not eligible to play.
- Females may play on male teams; males may not play on female teams.
- Premier players may not play on Medal Flight teams.
- A jersey must be provided to the guest player by the host team.
- International Homestay players may not double roster.
Double Rostered Players
- Players may roster on one team in addition to their primary team.
- Email registrar@usacup.org the names of double rostered players, team name and tournament ID numbers for both primary and secondary teams using the Double Rostered Players link.
- The player fee will be waived on the secondary team.
- Double rostered players must be listed on both the primary and secondary team rosters.
