Required Forms for Registered Teams Due June 1

Team Checklist

1. Complete the online tournament roster.

2. Direct parents to complete the online NSC Liability Waiver.

3.  Player fees are payable by Visa, MasterCard, check, money order or cash. Team payment only; no individual payments.

4.  Obtain guest player loan form for any non-Minnesota player on your team, if applicable.

5.  Required hotel lodging, if applicable.

6.  Player passes are required for play.

7.   Print the rules. Coaches are responsible for knowing the rules in advance. Express concerns prior to the start of each game.

8.  Print the Activities Packet. Check out all that USA CUP has to offer!
